Join the Karnataka Optometry Association
Membership is open to qualified optometrists and to optometry students. It connects you to continuing education, professional support and a state-wide network of colleagues.

Three ways to be part of the association
Choose the category that fits where you are in your optometry career.
Rs. 500
For qualified optometrists who prefer a term membership. Valid for three years and renewable.
Rs. 300
For optometry students. Requires the name and address of your optometry institute and your principal's details.
Documents required
What to enclose with your application
Please attach attested copies of the following with your membership form.
- Mark sheets of 10th and 12th standard (high school and senior secondary school).
- Attested copy of the diploma or degree awarded by your optometry institute.
- Each year's mark sheet from your optometry institute.
- The affiliation or registration of your optometry institute from the respective state medical faculty.
- An introductory reference from a senior or junior who is already a life member of any association (with their LM number).
- The approval or recognition letter for your degree or diploma in optometry, with the session of the course.
- A photocopy of the demand draft for the registration fee.
- Two stamp-size colour photographs for the ID card.
How to apply
Four steps to join
Fill the membership form
Complete the KOA membership form legibly, in block letters. Indicate the category you are applying for (life, associate or student).
Pay the registration fee
Pay by demand draft in favour of "Karnataka Optometry Association", payable at Bangalore. Note the draft number, date and bank on the form.
Enclose your documents
Attach the attested documents listed above and two photographs. Dispatch the form in an A4 envelope so the papers are not damaged.
Send to the secretariat
Post the completed application to the present secretariat: Expert Optics, #12 Nehru Main Road, Yadava Layout, Kammanahalli, Bangalore 560084.
Need the membership form? Email the secretariat at karnatakaoptometryassociation@gmail.com or contact us and we will send it to you. The association reserves the right to accept or reject any application.
Membership declarations
What every member agrees to
By applying for membership, you confirm the following declarations.
- That the information provided in the membership form is true and correct.
- That there is no legal or medico-legal case pending against you in any court in India or abroad.
- That you will not, in future, hold the Karnataka Optometry Association responsible for any misconduct during your practice as an optometrist or as an individual. It remains entirely at the discretion of the KOA office whether to assist or support you should such a situation arise.
- That you will immediately inform the KOA office of any change to your name, correspondence address or phone number.
